Preparing for Your Visit
Please complete the electronic forms via the patient portal once you are registered and scheduled for an appointment.
For Children & Teens:

Obtaining information from previous providers and communication with teachers and/or therapists is an important part of your child’s evaluation. Please print a consent form for each provider that needs to be contacted.

Your child’s teacher/counselor spends a significant amount of time with him/her and can provide valuable information on his/her performance and difficulties. Please consider authorizing your child’s teacher/s to fax/mail the forms directly to the office. Teacher consent form >>
For Transcranial Magnetic Stimulation (TMS) only:
If you have been referred or you are currently under the care of a mental health provider, we need to be able to contact them to discuss/update your progress. Please print a consent form for each provider that needs to be contacted.
Read Policies
Please read carefully the privacy and office policies so that you will gain an understanding of our office procedures prior to your first visit and we can get you on your way to recovery as soon as possible. Please feel free to call our office with any questions or if any clarification is needed. WBMA, LLC does not take any insurances, accepted forms of payment include cash, checks and credit cards (American Express, Visa, Mastercard).
Payment
We are out of network for services but provide superbills for our patients. Superbills contain information our patients need to submit to insurance companies for potential reimbursement of services. We ask that new psychiatry patients provide a $200 deposit toward their initial fees. If you need to cancel please do so within 48 BUSINESS hours of your scheduled appointment, and we will refund you the deposit. Failure to cancel within 48 BUSINESS hours may result in forfeiture of your deposit. Please click on the button below to access the payment options.
